One case.
Six clues.
One diagnosis.

Symptoms is a daily diagnostic puzzle for doctors and medical students. One new case a day, six clues, and a guess from a bank of around 1,200 conditions. You always learn the diagnosis and the symptoms behind it, plus a few things you didn't know about the condition. Come back tomorrow for the next one, or stay with it.

How it works

A new case, every morning. Always six clues.

Built around the diagnostic reasoning you already do, compressed into a single, satisfying daily ritual.

01

Read the presenting clue.

Each case opens with the chief complaint and patient demographic. From there, clues reveal in clinical order: history, exam findings, vitals, then investigations.

02

Guess the diagnosis.

Type to search 1,200+ diagnoses, then submit your best differential. The right guess wins the case. Wrong guesses file to your ruled-out list, with six ending the round.

03

Compare with peers.

See how many clues today's clinicians needed. Track streaks, distributions, and your ranking against thousands of others working the same case.

How cases are made

Every Symptoms case is written by a practising clinician and independently peer-reviewed before it's published, across internal medicine, emergency, paediatrics, obstetrics, and surgery. Cases are fictional — a puzzle, not a clinical tool.

Frequently asked

Questions, answered.

Who is Symptoms for?

Anyone can play. Symptoms is built around real clinical reasoning, so doctors and medical students use it to keep their skills sharp, and non-clinicians use it to learn how conditions actually present. Cases are pitched at internal-medicine speciality-trainee level, with a mix of bread-and-butter presentations and zebras.

It is educational, not a clinical decision tool. Cases are fictional and must not be used to diagnose real patients.

Is Symptoms free?

Yes. The daily case is free, supported by ads that are either relevant to you or from medical sponsors.

If you'd rather not see ads at all, you can go ad-free from £0.49/month or £4.99/year. Symptoms+ from £2.99/month adds the full archive, unlimited cases, speciality filters, and detailed write-ups.

When does the new case go live?

Every day at midnight in your account's time zone. You can play any time during the day; once the day rolls over, today's case moves to the archive.

Where do the cases come from?

Cases are written by practising clinicians across internal medicine, emergency, paediatrics, obstetrics, and surgery, then independently peer-reviewed before publication.

Do I need to verify I'm a clinician?

Not at all. Symptoms is open to anyone who wants to play, no verification needed.

Is my data shared with advertisers?

We store your guesses, scores, and case history in our cloud because the service needs them. None of it is shared with advertisers.

On the free plan, your iOS Advertising Identifier is shared with Google AdMob to serve you relevant ads. On any paid plan, nothing is shared.

When is it launching?

We're targeting an iOS launch in 2026, with Android to follow. Join the waitlist at the top of this page to hear about the launch.

I'm a clinician, can I write cases?
